The mission of our foundation is to provide financial support to hard-working, low-income families dealing with the expense of juvenile diabetes. Our Board of Trustees meets quarterly to consider all new applicants. The Board reviews donations, budgets and assigns the amounts to the families. In order to qualify, families must have the

following: (1) a child with juvenile diabetes, (2) low-income (3) full-time employment and (4) demonstrate a true need for financial assistance. Applicants may be required to provide income and other documentation in order to receive financial aid. The following is our Financial Aid Application.
